The Industrial Court has announced that the following forms for the commencement of prosecutions for offences under the Work Health and Safety Act 2011 in the Industrial Court of New South Wales from 1 July 2024 are now available here on the website for the Industrial Relations Commission of New South Wales:

Industrial Court Form 1 – Application to Commence Proceedings

Industrial Court Form 2 – Summons

Industrial Court Form 3 – Notice of Appearance
